How to Enroll

New Life Day School is a nonprofit organization dedicated to helping children from 6 weeks to 5 years old to thrive spiritually, academically, and socially.

New Life Day School is open year-round from 7:30 a.m. until 4:30 p.m., Monday through Friday, except for observed holidays and professional development days.

Application / Registration Fees

A $75 non-refundable donation fee must be submitted at the time of application.

REQUIRED FORMS AND DOCUMENTS

  1. Admission Information
    Obtain it after paying registration fees or download it from Form 2935.
  2. Health Statement
    Submit a medical statement signed by a health professional verifying your child’s readiness to participate, which is required before their first day.
  3. Diet/Allergy/Disability Forms
    If applicable, have your medical provider complete forms for diet modifications, allergies, or disabilities, including symptoms and any emergency medication (e.g., EpiPen).
  4. Financial Aid Application
    Inquire about financial assistance for families where BOTH parents are working or studying, based on income.
  5. Additional Required Documents
    • Child’s immunization record (up-to-date per Texas state guidelines)
    • Birth certificate
    • Parent/guardian photo ID
    • Proof of residency
  1. Physician’s Diet Modifications/Allergies/Disabilities and FARE (Food Allergy & Anaphylaxis Emergency Care Plan)
    If your child has any allergies or disabilities, these forms must be completed by a medical provider. Provide detailed information, including symptoms to monitor and any emergency medications (e.g., EpiPen) provided.
